All the Annual Permission Forms are completed online. A link to the forms is emailed to parents/guardians before the start of each new school year. The following forms can be found below on this page and also in the LPS Student & Family Handbook

• Medication Authorization Form
• Permission to Publish Student Image and Work
• Release to Media Outlets
• Student Use of Third-party Software Services
